We are so excited to announce this contest for a 2nd year! All children zero to eighteen are eligible to participate. The best part is all you have to do is show up on July 14th with your smile ready to be
photographed by Lasting Images photographer Anne-Marie Shumate. It’s that easy!
There is a $5.00 non-refundable entry fee per participant in which 100% of the proceeds will go to the
Child and Family Guidance Center of Texoma. We, at Pediatric Dental Specialists PA, are very excited to support and raise awareness about this great organization. We hope you will join us by being a part of this fun event!
*Every participant will receive one free 5x7 from Lasting Images Photography. (Image will be chosen by the photographer)
*Nine finalists will be chosen, three from each of the following age categories: 0-3, 4-10, 11-18.
*The nine finalists will each receive a $150.00 gift certificate from Lasting Images Photography.
*Each age category will also have a grand prize winner. Fabulous grand prizes will be awarded!
Please complete the online registration form and make sure to fill in all fields, pay the entry fee and submit. If you are in the office and/or would like to register in person please fill out a registration form and pay your entry fee at check-out (only cash can be accepted in the office) You will be notified by phone of your appointment time. Each participate/guardian will need to complete a Model Release Form upon arrival on July 14th.
You will have approximately 5 minutes with the photographer so please come ready to go. The photographer has suggested a white shirt or other solid color top for best pictures.
